Blood pressure management is an essential part of
maintaining the health, safety, and wellbeing of people living in care homes.
Many residents are older adults with long-term health conditions, frailty, or
cognitive impairment, all of which can increase the risk of high or unstable
blood pressure. Because changes in blood pressure often occur without obvious
symptoms, regular monitoring and appropriate management are vital in preventing
avoidable harm.
In care home settings, effective blood pressure management
supports the early identification of health concerns, reduces the risk of
serious events such as strokes, heart attacks, and falls, and contributes to
person-centred care. It also plays an important role in supporting safe
medication use and informed clinical decision-making in partnership with GPs,
community nurses, and other healthcare professionals.
This document provides guidance on the monitoring,
recording, and management of blood pressure within care homes. It aims to
support staff to deliver safe, consistent, and compassionate care, while
recognising the need for individualised approaches that respect each resident’s
health needs, preferences, and quality of life.
